Chula Vista Elementary School Online Enrollment

 

Welcome to Online Enrollment!

Please read the following information carefully. If you have any questions about enrollment, please call:

Note: Continuing students DO NOT need to re-enroll.

 

1. GET READY

IMPORTANT: UNTIL ALL DOCUMENTS ARE RECEIVED BY YOUR CHILD'S SCHOOL SITE, THE ENROLLMENT PROCESS IS NOT COMPLETE.

Gather the following documents to take to your child's school. These will be needed to complete the enrollment process, after you have completed Steps 2 and 3.

  • Birth certificate document for the student (click for acceptable documents)
  • Child's immunization record (click for requirements)
  • Photo identification of the parent/guardian registering the student(s) and,
  • Residency Verification document (click for acceptable documents)

Note: School of Attendance is based on your current residential address. If you do not know your school of attendance, the Online Enrollment procedure will help you identify the school. For more information, click here.

2. GET SET

  • Click ONLINE ENROLLMENT to begin. Note: The anticipated start date for students enrolling for the 2017-2018 school year is July 19, 2017.
  • Be sure to click the 'SUBMIT' button to enter your information, and PRINT.
  • In addition to the Online Enrollment information, you will also need to complete the following forms (in green text below). Fill out and print each of the forms.
  • You will need the latest, free version of Adobe Reader to download these forms. Click here to download. Be sure to uncheck any 3rd Party add-ins.

    1. Emergency and Health Information English / Spanish
    2. Important Health Issues English / Spanish
    3. Home Language Survey English / Spanish
    4. Record of Prior School Programs and Special Services English / Spanish
    5. SDIR Parent Information Form English / Spanish
  • If you are enrolling your child in Kindergarten or 1st grade, you will need to have the following forms completed by your child's physician and dentist.
    1. School Entry Health Checkup (required to enter 1st grade) English / Spanish
    2. Oral Health Assessment (required on/before May 31 of a child's first year in public school) English / Spanish

You are ready to proceed to your child's school of residence!

Take all of the above documents to your school site to complete the enrollment process.
